Covering Letter : Covering-letter from applicant on company’s letter head mentioning his name, designation, passport number, purpose and duration of visit. The letter should be addressed to – The Visa Officer, Embassy of the Slovak Republic, New Delhi.

Invitation Letter: Original Letter from Slovak Company duly attested by the Slovak Foreign Police.

(b) Original Company and Personal Bank Statement for last six months mentioning the applicant’s name and address with Bank Seal & Sign. Note :Bank statement Should be on A4 Size Paper.

Airline Reservation : Ticket Itinerary

Medical : Original Medical Insurance for the stay duration amounting to 30,000 Euros

Authorization Letter: Applicant will have to submit an authorization letter authorizing Insight Vacations Private Limited to submit and collect documents.

 Personal appearance is required to submit the application & Embassy will advice the date of biometrics at the time of submission.
