Covering Letter from applicant on company's letter head stating his name, designation, passport number, purpose and duration of visit, duly signed by authorized signatory and addressed to:

| Invitation Letter |
Invitation letter (Approval letter) in Arabic language from the Ministry of Iraq.
One copy to be faxed to the Embassy.

| Medical |
a) Medical report is required to be submitted for the tests of CBC, HIV, Hepatitis and Vaccination for Malaria issued by Government's Laboratories / Hospitals.
HIV report is necessary to be attested by the Ministry of External Affairs, Government of India.
Note - Post MEA attestation, HIV report will also be attested by Iraq Embassy. It must be carried by the applicant while traveling to Iraq. Embassy attestation fee is USD 25.
b) Applicant must provide a report of Yellow fever vaccination. |
