Passport: Original passport with a validity of more than six months and minimum two blank pages for visa stamp. Attach all your old passports (if any)

Dominican Republic Visa Application Form : One visa application form to be duly filled and signed by the applicant.
Photo Specification: One recent passport sized colored photographs on a white background with 80% face cover on a matt or semi-matt finished paper (35mm x 45mm)
Please note: Photograph should not be more than 3 months old, scanned/stapled and should not be used in any of the previous visas.
Covering-Letter: Covering Letter from applicant on business letter head mentioning name, designation, passport number, purpose and duration of visit in brief. The letter should be duly signed by authorized signatory with company stamp and addressed to – The Visa Officer, Embassy of the Dominican Republic, New Delhi.
Please  Note  : Covering Letter on Letterhead if applicant is Self Employed or on plain paper if Employed .Invitation Letter: An Invitation letter from Dominican Republic host (friend / relative) along with copy of the identity / resident card of the invitee and invitation letter should be notarized and legalized by Procuraduría General de la Republican  Dominican .
Police Clearance Certificate: PCC certificate is required.
Please Note: PCC has to be issued from passport office and should not be more than six months old.
Financials:
a) Letter from the Bank providing - Date of opening of the account, annual average balance maintained during the year, date of last deposit. (No Bank Statements)
Airline Reservation: Ticket Itinerary
Proof of Accommodation: Hotel Confirmation
Please Note:
a) All the supporting documents (Covering Letter, PCC, Bank Letter, Medical & Birth Certificate etc) must be submitted in Spanish or along with its "Spanish Translation".
b) If the applicant has valid multiple entry visa for USA,  Canada , UK or Scheme  countries then he/she can obtain tourist card on arrival or can apply in the Embassy for the tourist visa.
c) Only a passport with a valid visa for USA, UK, Canada and Schenegen is required to obtain tourist card on arrival if traveling only for tourism purpose. This tourist card has to be purchased at immigration on arrival at a cost 10$ (approx) which allow the traveler to stay for 30 days only.

Police Clearance Certificate- From the country, where applicant is residing from the last 6 months- apostle from Ministry of Foreign Affairs of resident country (PCC should not be more than 6 month old).

Medical : Original Medical Certificate of applicant certifying that he/ she is not suffering from any contagious diseases and legalized by Ministry of Foreign Affairs of resident country and further legalized by Embassy
